I would stop sulfonylurea due to risk of hypoglycemia and DPP IV. I would continue metformin and add a GLP 1 to help with weight loss and cardiovascular protection.

    I would discontinue the Sulfonylurea and DPP4 since she is not achieving the goal, I would recommend putting her on SGLT2 Inhibitor which has proven benefits on the kidney, CHF, Show More
