Cognitive Decline in Early and Premature Menopause

Cognitive Decline in Early and Premature Menopause

Source : https://pubmed.ncbi.nlm.nih.gov/37047549/

Early and premature menopause, or premature ovarian insufficiency (POI), affects 1% of women under the age of 40 years. This paper reviews the main aspects of early and premature menopause...

Early menopause increases cognitive decline risk; menopausal hormone therapy may reduce mild cognitive impairment and Alzheimer's risk. Multidisciplinary care is crucial for mental health in affected women.

Obesity and menopause

Source : https://pubmed.ncbi.nlm.nih.gov/37244787/

The global obesity pandemic continues to rise, with figures from the World Health Organization showing that 13% of the world's adult population was obese in 2016. Obesity has significant implications,...

Understanding this complex interplay of obesity and menopause is important to providing the right advice/management. We review the current evidence on obesity and menopause, focusing on the implications of increased obesity during menopause, the impact of menopause on obesity, and the effect of available treatments on associated morbidities.

The aim of this review was to provide an updated assessment of the present diagnostic tools and clinical symptoms and signs to evaluate uterine fibroids (UFs) on the basis of...

Uterine fibroids (UFs) are common and often asymptomatic. TVUS is the initial diagnostic choice for its sensitivity and accessibility. MRI complements TVUS, especially in complex cases. Emerging technologies like SIS, elastography, CEUS, and AI show promise in improving diagnostic accuracy and efficiency for UFs.

Gynotoxic Effects of Chemotherapy and Potential Protective Mechanisms

Gynotoxic Effects of Chemotherapy and Potential Protective Mechanisms

Source : https://pubmed.ncbi.nlm.nih.gov/38927992/

Chemotherapy is one of the leading cancer treatments. Unfortunately, its use can contribute to several side effects, including gynotoxic effects in women. Ovarian reserve suppression and estrogen deficiency result in...

Women of reproductive age undergoing chemotherapy are at risk of irreversible ovarian failure. Current methods of determining the ovarian reserve do not accurately predict the future reproductive potential of patients undergoing chemotherapy. Among the most ovotoxic cytostatics are alkylating agents, mainly cyclophosphamide, but the adverse...

Uterine Tumor Resembling Ovarian Sex-Cord Tumor (UTROSCT): A Rare Polyphenotypic Neoplasm

Uterine tumor resembling ovarian sex-cord tumor (UTROSCT) is a rare form of uterine mesenchymal neoplasm. Although UTROSCT generally exhibits benign behavior with a favorable prognosis, this neoplasm is nevertheless classified...

Through our review of the literature, we have determined that UTROSCT is a rare neoplasm. In our opinion, its pathological diagnosis can be difficult for many reasons. First, the neoplasm is rare, and often the diagnosis must be made by an expert in the field. In addition, due to the many histological features that the lesion can show, many...

Background: Osteopenia, caused by estrogen deficiency in postmenopausal women (PMW), lowers Bone Mineral Density (BMD) and increases bone fragility. It affects about half of older women's social and physical health....

Both groups adhered to the SIOMMMS guidelines, participated in daily outdoor walking for 30 min, and demonstrated improved physical performance and quality of life. However, the experimental group, who received Kinect-based VRT in addition to walking, exhibited more substantial enhancements compared to the control group. The findings suggest...

Estrogen-immuno-neuromodulation disorders in menopausal depression

A significant decrease in estrogen levels disrupts immune homeostasis, particularly the levels of the immune factors TNF-α, IL-1β, IL-4, IL-6, IL-10 and IL-18 through the ERα/ERβ/GPER-associated NLRP3/NF-κB signaling pathway, which involves many signaling molecules, such as TLR2, TLR4, P2X7R, SIRT1, CREB, MS-KIF18A, PI3K, AKT, Gs, cAMP,...

Although the loss of sex hormones (estrogen, progestogens, and androgens) due to natural or iatrogenic menopause can impact sexual function, checking hormone concentrations is not recommended and should not be used to guide treatment.

Novel MOA targets age-old condition

Hot flashes—experienced by 70%-80% of menopausal women—are physically distressing and worsen quality of life. Newer therapies, such as NK3R antagonists, leverage a non-hormonal approach to addressing these symptoms.

For example, fezolinetant is a selective NK3R antagonist that directly blocks NKB from binding to the KNDy neuron. NKB is a known trigger of vasomotor symptoms (VMS).

KNDy neurons located at the level of the hypothalamus are inhibited by estrogen and triggered by the neuropeptide NKB, thus resulting in the regulation of body temperature.

During menopause, estrogen decline interferes with this balance involving NKB. Left unopposed, NKB signaling increases KNDy neuronal activity. This process sets off heat-dissipation mechanisms, such as vasodilation and sweating (or VMS).

Thus, fezolinetant helps decrease the frequency and intensity of hot flashes and night sweats, honing in on NK3R with an affinity 450 times higher than that of NK1 or NK2 receptors.

What are your thoughts on this new mechanism of action for the treatment of VMS?
